Arond-Thomas, M.D.FROM: CCM RE: Opt-out Status I am writing to you to inform you of my decision to change my status in relationship to Medicare billing arrangements. Medicare offers doctors three billing options. Physicians may sign a participation agreement and accept Medicare’s allowed charge as payment in full. They may elect to be a non-participating physician, which permits them to bill patients for somewhat more than the Medicare allowance. Or they may become a private contracting physician agreeing to bill patients directly, in which case neither the doctor nor the patient may receive any re-imbursement from Medicare. It is my intention to develop a private contract with patients eligible for Medicare who seek CCM services, and I am filing an affidavit with Medicare stating that I agree to forgo receiving any payment from Medicare for items or services provided to any Medicare beneficiary for the following 2-year period. Each beneficiary will be asked to sign a contract, and it should be understood that Medicare will not pay for the services provided under this contract. It is also understood that this contract cannot be entered into at a time when the beneficiary is facing an emergency or an urgent health situation. By signing this private contract, the beneficiary: 1. Gives up all Medicare payments for services furnished under my new “opt out” status. 2. Agrees not to bill Medicare or ask CCM to bill Medicare 3. Is liable for all of my charges, without any Medicare balance billing limits 4. Acknowledges that Medigap or any other supplemental insurance will not pay toward the services 5. Acknowledges that he or she has the right to receive services from physicians for whom Medicare coverage and payment would be available. I am taking this step so that I can focus primarily on providing my innovative health promotion and cancer prevention program, Morph2Health®, to ALL patients and clients of CCM. This program makes use of my 20 years of research on the pivotal informational role that the autonomic nervous system (ANS) plays in the way that your genes express themselves, both in the promotion of health and well being, as well as in the production of various cancers. Our mission is to educate and empower those who use our programs to be active participants in a process called healing, rather than being solely a patient in an experiment in search of a cure. Each of these 5 dimensions is subject to dysfunction in the presence of toxic waste by-products when they are removed poorly, or, more detrimentally, when they are stored within 1 or more of the 5 dimensions. The end result of this cumulative multi-dimensional dysfunction is a failure in the control over the life/death cycles of cells that subsequently become cancerous. Your autonomic nervous system is the regulator of the internal chemical environment of each body organ, and is directly responsible for removal of toxic by-products. In this capacity, dysfunction within the autonomic nervous system is a modifiable factor that can influence the way that the body regulates it health, fights off aging, and reduces your risk for developing diseases of aging such as Alzheimer’s. More importantly, re-education of the way that your autonomic nervous system works prevents and reverses the 3 stages of cancer development: initiation, promotion, and progression.
